With a worldwide influenza epidemic sweeping through the city (commonly called the Spanish Flu), the hospital was immediately filled to capacity. Soon, the daily census had passed 100, and patients were housed on the verandas. The bed capacity was supposed to be from 30 to 35, but by 1920 it was running from 50 to 60.

New Miami City Hospital Opens
On June 25, the new Miami City Hospital opens. Miami’s first physician, Dr. James M. Jackson, served on the advisory board for its construction. The east administration building (now known as "the Alamo") featured consulting rooms, operating rooms for minor surgery, waiting rooms, business offices, and restrooms on the first floor. Rooms for private white patients (13 beds) were on the second floor, and the third floor was reserved for storage. A drug room, kitchen, and dining room were located in a service building in the middle, while the building on the west side had capacity to care for 10 male and 10 female white patients in wards. There were also four isolation rooms. Several wood houses were also remodeled to serve as operating rooms, a nurses’ home, and a ward for black patients.

As consultant Frances A. Stone had predicted, the hospital’s location “too far out in the country” hampered its operation. So when Ainah Royce (in the passenger seat) started her nursing school, she made sure they had use of a car.

The Miami Mayor appointed a nine-member volunteer Board of Trustees in November. The hospital leaders hired Ainah V. Royce, RN, a Florida nursing administrator to act as superintendent. When Royce arrived in December, the hospital had 55 patients crowded into the wards and lying on cots on the porches. The total staff was 13, including one doctor in residence.
